Improving the quality of HIV/AIDS services

Interaction of AIDS/TB Services

In the framework of providing technical assistance in the development of AIDS/TB Interaction Model, in July, 2005, work meeting with participation of MoH, AIDS and TB services representatives, together with other government and non-government organizations, and mass media, was held. During the meeting, a TWG was established, which included representatives of both services, MoH, WHO, GF/TB, HOPE/TB and other international organizations. The TWG defined pilot regions for the Model implementation (Tashkent city and Tashkent oblast) and developed the following documents:

  • Draft MoH Order on AIDS/TB Model realization in pilot regions;
  • Methodological recommendations on detection, registration, treatment and prophylaxis of TB in HIV-infected patients in the Republic of Uzbekistan;
  • Perspective plan and the Model realization indicators.

The package of the documents developed was approved by the Ministry of Health of Uzbekistan. In correspondence with the plan of AIDS/TB Model realization and under the CAPACITY Project technical assistance, the following activities on improvement of interaction of AIDS/TB Services were conducted:

  • Orientation meetings for health managers of both services provided;
  • National group of trainers on interaction of AID/TB services trained;
  • More than 200 medical specialists trained on interaction of AIDS and TB services;
  • Trainings on M&E of the Model realization conducted;
  • National group of experts on providing M&E activities selected out of the trained specialists;
  • Two steps of M&E activities conducted by M&E expert group, revealed successes and problems in the Model realization;
  • Roundtable and TWG meeting on the results of AIDS/TB Model realization in pilot regions and development of recommendations for MoH consideration with the purpose of further dissemination of the Model in other regions of Uzbekistan conducted.

HIV/TB and ART Course for Tashkent Institute of Advanced Medical Education (TIAME)

Under the CAPACITY Project technical assistance, a training course for TIAME teachers was developed with the purpose of its integration into TIAME curriculum. Besides, as per the request of health care specialists, film describing HIV replication used during the trainings, was adapted into Russian and Uzbek languages for future use in the framework of HIV/TB curriculum of TIAME. This Course includes issues on dual HIV/TB infection and major approaches to providing ART in Uzbekistan. The Course was approved by TIAME scientific council and included to the curriculum.

In June, 2006, under the CAPACITY Project technical assistance, a ToT on interaction of AIDS/TB services and providing ART in Uzbekistan was conducted for TIAME teachers. After the ToT, the TIAME teachers started to teach medical specialists using the developed Course and some additional materials, provided during the ToT (National Protocol on providing medical care to HIV patients in Uzbekistan, training presentations, “HIV replication” film, which was translated from English into Russian and Uzbek languages, etc.)

Monitoring and evaluation of the AIDS/TB Model implementation

For evaluation of the realization of AIDS/TB Interaction Model M&E seminars for medical specialists of both services, including health managers, were conducted. During the M&E seminars, national group of specialists developed annexes to registration forms, M&E indicators and mechanism. Also, the group of specialists for providing M&E activities was defined.

In December, 2005 and in May, 2006, the M&E group conducted two steps of M&E activities in health care institutions of Tashkent city and Tashkent oblast. The purpose of these activities was to collect information on the Model realization progress (using the developed M&E forms: reports, questionnaires for doctors and patients, annexes to the registration forms). After providing these activities, M&E group made a detailed analysis of the M&E results that were presented at the roundtable on Interaction of AIDS/TB services and discussed during the TWG meeting. Besides, the roundtable participants and TWG members discussed problems occurred during the Model realization and possible solutions. As a result of the discussion, a number of recommendations on further steps of the Model realization in pilot regions were developed. These recommendations were submitted to the Ministry of Health for consideration and approval.

Antiretroviral Therapy (ART)

In October, 20o5, the CAPACITY Project provided technical assistance to RAC and PIU/GFATM in conduction of orientation meeting for health care managers of the Republic of Uzbekistan and training on major approaches to ART for doctors and nurses, involved into providing ART in the country. With this purpose, the CAPACITY Project hired a group of consultants from Ukraine and Kazakhstan who took part both in the orientation meeting and in the training.

By the moment of the beginning of ART in Uzbekistan (April, 2006), medical specialists of all levels received the first stage of training, which mostly included theoretical ART aspects.

In May, 2006, after ART drugs were delivered to Uzbekistan, under the CAPACITY Project assistance, consultants from Ukraine and Kazakhstan were invited to conduct the second ART training (clinical). This training was provided in cooperation with RAC and Virology Institute, where the trainees were given an opportunity to work with patients. Thus, the clinical part consisted of the detailed analysis of clinical cases, consultations and examinations of patients. Issues discussed: diagnosis, differential diagnosis, examination plan, treatment plan, possible problems and complications. Special attention was paid to the ART initiation (prescription, laboratory evaluation, regimes and surveillance).

In the result of ART trainings, 50 medical specialists (doctors, nurses) were trained, and now they are directly involved into providing ART in Uzbekistan.

ART Conference organized by PIU/GFATM

In May, 2006, the CAPACITY Project assisted PIU/GFATM in conduction of ART Conference on review of ART trainings results, development of recommendations and further steps. During the conference, the participants (health managers from Oblast AIDS Centers, MOH representatives, RAC, WHO, PIU GF, Pediatrics Institute, Obstetrics and Gynecology Institute, etc.) discussed National Standards (Protocols) of providing ART by health care specialists, interaction of AIDS/TB Services and importance of providing TB treatment before starting ART, providing ART in children, vertical transmission, ART drugs supply and other important issues.

As a result of the conference, it was decided/suggested to provide a separate training for pediatricians on HIV problem among children, conduct additional seminars on VCT for obstetrician-gynecologists due to the urgent problem of vertical transmission, purchase express tests and provide maternities with nevirapin.

Training on Materials Development organized by HOPE TB Project

In February, 2006, the CAPACITY Project/Uzbekistan team took part in the training organized by HOPE TB Project with support of Johns Hopkins University. During 3 days of training, a number of presentations on promotional materials was developed for World TB Day, a lot of products were developed (radio spots, posters, booklets, brochures, TV promotions, etc.) and finally almost all of them were pre-tested among target groups: patients, doctors, nurses and general population. CAPACITY Project/Uzbekistan was included into the TWG on TB information and educational work.
