Improving the Quality of HIV/AIDS Services
Interaction of TB and HIV/AIDS services With a purpose to promote continuum and quality care and treatment of HIV/AIDS and TB co-infection, CAPACITY has developed and tested the pilot project on establishing and strengthening the interaction of TB and HIV/AIDS services in Chui Oblast on the basis of medical (CFMs, territorial hospitals, AIDS and TB Centers) and penitentiary institutions located in Chui Oblast. During the course of the pilot implementation the following has been achieved: a package of normative legal documents is developed (methodical guidelines, monitoring and evaluation plan for TB and HIV/AIDS services, data collection forms and etc); the joint Prikaz (‘Order’) of the Ministry of Health and Ministry of Justice on the measures of amplification of the struggle against HIV/AIDS is issued; pool of trainers and monitoring team are set up to serve as a resource for scaling up the model around the country; TB/HIV component is integrated into the training process of the Kyrgyz Medical Institute of Retraining and Professional Skills Enhancement. Eventually, implementation of the model resulted in defining of the set of organizational, clinical, economic, social and other measures aimed at developing an accessible, quality and socially oriented system of interaction between structural subdivisions of the Public Health (HIV/AIDS and TB). Currently, CAPACITY is working towards the scaling-up of the model to other regions of the country.
Voluntary Counseling and Testing (VCT) Within the framework of the State Programme on HIV/AIDS, CAPACITY is providing technical assistance in developing the VCT system with elements of provider-initiated consulting and testing in Kyrgyzstan. Thus, in March 2008, CAPACITY consultants developed the VCT model for two pilot sites: Kara-Suu and Osh. The model describes the most effective mechanisms of interaction among governmental and non-governmental institutions that provide VCT and HIV services. The main purpose of the model is to raise the public awareness about HIV, decrease stigma and discrimination and increase an accessibility of at most risk populations to HIV services. The implementation of the model is commenced in the above regions that have already been involved in the 60+ TUMAR prevention model for at most risk populations (IDUs and SWs). For a successful implementation of the model CAPACITY together with local partners will conduct information campaign, establish coordination mechanism and develop legal tools, set up and develop friendly service for at most risk populations and establish the system to monitor and evaluate the VCT model. Experience and lessons learned from this pilot will be disseminated among national partners for further work in the field of VCT development.
Integration of the Substitution Therapy by methadone in Penitentiary Institutions of Kyrgyzstan CAPACITY provided organizational and financial assistance to the Ministry of Justice in conducting the Round Table entitled "Integration of the Substitution Therapy by methadone in the Penitentiary System in Kyrgyzstan”, which took place in Bishkek on February 1, 2008. During the meeting the representatives of the Office of the Prime-Minister, ministries and departments, international, non-governmental and other public organizations reviewed the presented pilot project on introduction of Methadone Substitution Therapy in penitentiary system and discussed the issues related to HIV/AIDS prevalence in penitentiary institutions in the Kyrgyz Republic. It should be noted that such programme has already been successfully integrated in the civil health care system. As a result, there were observed positive trends in the social adaptation of drug users and decrease of the risk of dissemination of HIV through IDUs. Currently, the implementation of the pilot project with financial support of GFATM is commenced in the penitentiary institutions located in Chui Oblast and Bishkek city. CAPACITY is planning to provide further support while this project implementation.
